Electrocution investigation shows insulation failure - 7/3/2003
 

An Energy Safety investigation into the electrocution deaths of two children in Wyndham on Monday has revealed a failure in the insulation of the cable connecting the house to a power pole.

Director of Energy Safety Albert Koenig said today it appeared that the children’s Frisbee had become caught in an anchor bracket near the edge of the roof of their home.

“The Energy Safety inspector who investigated the incident found that the metal cable fitting had slipped off the insulator on which it normally rests, resulting in the bracket becoming live,” Mr Koenig said.

“It is believed the children climbed onto the metallic roof to retrieve their Frisbee and both touched the bracket and received a fatal shock when the metal roof provided a circuit to earth.

“This tragic incident has highlighted two safety issues. Firstly, that nobody should attempt to retrieve objects near electricity cables or touch the brackets supporting these cables.

“Secondly, the incident highlights the importance of electricity suppliers correctly maintaining electrical distribution equipment, especially by ensuring that aerial service cables are maintained in a safe and serviceable condition.”

Mr Koenig said that his office would continue to work with the Police Service to complete the coronial enquiry, and would provide a formal report to the State Coroner’s Office.

He also said he would separately investigate whether there had been any breach of safety regulations by Western Power.
